ID Day liability release, photo release and health acknowledgment

I waive and release Premier Soccer Company LLC, Ashland University, and their staff from all liability for injuries that occur at the ID Day. All medical expenses will be the responsibility of the above minor and the minor's parents and/or guardian. Any items that are lost or stolen are not the responsibility of Premier Soccer Company LLC, Ashland University, or their staff. Any pictures or video taken at the event may be used by Premier Soccer Company LLC and Ashland University for promotional purposes.
